The next step in your volunteer enrollment is the completion of required forms. These forms can be opened and printed to fill out and return to us. They can be dropped off at the Health Center Main Entrance or mailed to Volunteer Coordinator, Stonehill Communities, 3485 Windsor Ave, Dubuque, IA 52001.

COVID Vaccine Exception Forms - Individuals who are not vaccinated for COVID-19 must complete an exception form.
Once we have received your paperwork and your background check has been completed, we will reach out to you to schedule a tuberculosis (TB) test. After your first TB test has been read, you can begin volunteering. The department director or coordinator will be in contact with you to schedule a start date after the first TB test.
