COVID-19 Booster Consent Forms

We are preparing to offer second COVID-19 vaccine boosters to residents. Resident Emergency Contacts for whom we have email addresses have received this request via email from us using the DocuSign system for electronic signature. This allows you to quickly and easily provide your consent or decline the booster for your loved one. If you do not see the Consent in your email and need a paper copy, please contact us at 563.557.7180.

If you have any questions regarding visitation or our ongoing precautionary measures you may contact Peggy Stockel, V.P. of Operations or Matt Jahn, Director of Health Services at 563-557-7180.

Thank you,

Peggy Stockel
