We have identified 3 new positive cases of COVID-19 in Health Center residents and 1 new case in an employee and have additional tests pending. There are currently 18 positive cases of COVID-19 in staff and residents. We are in Outbreak Status. Affected households include: 1st Floor Skilled Household, 1 Southeast, 2 Southeast, 2 Northwest, and 3 Northwest. Visitors are allowed for all residents.

COVID-19 Booster Consent Forms

We are preparing to offer second COVID-19 vaccine boosters to residents. Resident Emergency Contacts for whom we have email addresses have received this request via email from us using the DocuSign system for electronic signature. This allows you to quickly and easily provide your consent or decline the booster for your loved one.
