Eligibility Requirements

Stonehill Adult Center will provide equal access and services to all without regard to race, color, national origin, or disability.

Persons participating in the adult day center program criteria:

• Be age 60 or older; being cared for at home or living alone. Younger adults will be accepted if, after a trial period, the director and staff believes the individual will benefit from the program.
• Have a physical, social, emotional or memory impairment for which the individual can receive benefit from a supervised, stimulating and protective environment.
• Be able to walk independently, or with the assistance of one other person, or use of a wheelchair, cane or walker.
• Be able to eat independently or with the assistance of one other person.
• Be able to independently transfer from sitting to standing or able to transfer with the assistance of one other person.
• Be continent, or if incontinent, managed with a toileting schedule or incontinent aids.
• Be able to function in a small group setting without continuous one-to-one attention and be able to communicate needs to staff.
• Be free of communicable and infectious diseases.

All participants must have an application with emergency information and copies of any advance directives on file.

In the event a cardiac arrest would occur or the need for medical treatment beyond our capabilities 911 will be notified and emergency transfer will be conducted if needed.
